Are there any forms I need to fill out to receive my payment?

Yes - please log into your account to access the required forms.

Once logged in, click on the 'Settings' icon  in the upper right hand corner. You will see 3 tabs: Payment Information | Tax Information | Account Information.

Click on the 'Tax Information' tab. Select the appropriate tax form. A PDF form will appear. Please print and complete the form. You can scan and email or fax it to us (detailed instructions are provided on the 'Tax Information' page).
